• Use this form for artwork created by up to two students. If more than two students have collaborated, please use the Multiple Artists Label Form.
• Please type all information exactly as you wish it to appear on your display label.
• Submit only one form per school; please coordinate with the other art teachers at your school.
• Deadline for submission: October 14, 2011.
